Purpose

WalgreensFail documents a cross-location pattern of pharmacy and clinic failures at Arizona Walgreens locations and the Walgreens-owned Village Medical — from dated correspondence and formal complaints to the Arizona State Board of Pharmacy and the Ombudsman-Citizens’ Aide. The point is the pattern: the same categories of failure recurring across different stores.

Scope

This site covers Arizona Walgreens pharmacies and the affiliated Village Medical / VillageMD and Summit Health. It does not cover other pharmacy chains, and it excludes an older out-of-state matter that does not fit this family’s Arizona focus. Noncommercial & public-interest

This is a noncommercial project. It sells nothing, solicits no donations, and runs no advertising. It is published in the public interest as documentation and commentary on the conduct of a large pharmacy chain and its affiliated providers — a matter of legitimate public concern.

Every documented claim is tied to a specific source — dated correspondence, or a complaint reference number (Board of Pharmacy Case #838441; Ombudsman Complaint #2601100). Nothing is published from memory, inference, or general reputation. Complaints filed with a regulator are shown as filed and acknowledged; the site does not state or imply that the Board of Pharmacy substantiated or dismissed anything — as of the record, it has done neither. Privacy

The patients involved — including the author’s own family members — are referred to by relationship (“the complainant,” “his wife,” “his father”), not by name, and identifiable medical detail is minimized. Corporate, affiliated-provider, and agency contacts named in the record are identified; front-line pharmacy staff are referred to by role.
