Each timeline entry and case claim is tied to something checkable — dated correspondence or a complaint reference (Board of Pharmacy Case #838441; Ombudsman Complaint #2601100). If we can’t source it, it doesn’t go in the record.
An incident described in the correspondence is presented as reported — what the patient or family documented at the time — not as an established finding.
Complaints filed with the Arizona State Board of Pharmacy and the Ombudsman-Citizens’ Aide are shown as filed and acknowledged. As of the record, the Board had not issued written outcomes on the submitted complaints. This site does not state or imply that any regulator substantiated or dismissed a complaint — open matters are marked open.
