A documented pattern of prescription-handling, patient-record, and public-health-policy failures across multiple Walgreens Arizona locations and the Walgreens-affiliated Village Medical, June 2025 to February 2026, with formal complaints to the AZ State Board of Pharmacy and the Ombudsman-Citizens' Aide (Complaint #2601100). What the record shows

Beginning with a 10-day delay in a heart-medication refill for the complainant’s wife through Village Medical (June 2025), the pattern recurred across four Arizona locations over the following nine months: a temporary-closure / prescription-access failure in Coolidge (Jul 2025), a phone-number-verification dispute during a vaccine visit in Coolidge (Sep 2025), a duplicate patient profile and a drive-thru refusal to a symptomatic patient in Coolidge (Dec 2025), a false “ready for pickup” notification tied to that same complaint, a lost / misplaced prescription for a heart-failure patient (the complainant’s mother) at the 7th St & Glendale Ave location in Phoenix (Feb 2026), and a second false “ready for pickup” notification affecting three prescriptions at 7000 N 16th St, Phoenix (Feb 2026), where the complainant also observed four other customers being told the same thing.

Complaints were filed with the AZ State Board of Pharmacy, AZDHS (which redirected to the Board, as pharmacies are outside its jurisdiction), Pinal County Public Health (which also lacks jurisdiction), the AZ Ombudsman-Citizens’ Aide, Walgreens corporate (CEO Mike Motz, District Manager John Piellucci, the WCR Executive Response team), and — on Jan 31, 2026 — individually to roughly 30 members of the Arizona State Legislature.

The Board of Pharmacy’s Deputy Director stated on Feb 4, 2026 that both complaints filed to that point had been received, acknowledged by email the same day each was filed, and were under investigation — a claim the complainant disputes, stating he received no acknowledgment. Both accounts are preserved here rather than resolved in either direction. As of Feb 25, 2026, the Board had agreed to open an additional complaint specific to the 7000 N 16th St location but had not provided written confirmation of findings, corrective action, or closure for any complaint. Status is open / monitoring; nothing here states or implies the Board substantiated or dismissed anything — it has said neither.

The systemic gap this exposes

Neither AZDHS nor Pinal County Public Health has regulatory authority over pharmacy operations — that sits with the AZ State Board of Pharmacy alone — which created a multi-week referral loop before the complaint reached the agency with jurisdiction. Separately, the Ombudsman-Citizens’ Aide’s own account of its authority confirms it cannot compel the Board to investigate, find fault, or report outcomes on a fixed timeline; it can only facilitate communication after the complainant has exhausted the Board’s own process.
