False “ready for pickup” notifications reported at two Arizona locations — a Coolidge-area prescription (Jan 27, 2026) and three prescriptions at 7000 N 16th St, Phoenix (Feb 25, 2026), where the complainant reported at least four other customers describing the same experience.
Prescriptions for a heart-failure patient (the complainant’s mother) reportedly accepted and then unlocatable on return, recovered only after the family insisted (7th St & Glendale, Phoenix).
A duplicate patient profile reportedly created without consent, separating a new prescription from the existing account; staff said the profiles could not be merged. During a drive-thru visit while the complainant was ill, staff required in-store entry for an over-the-counter item; a shift lead declined to name the filling pharmacist when asked for a regulatory complaint (Coolidge, Dec 2025).
The complainant’s framing throughout is that Village Medical / VillageMD and Summit Health, though operated with day-to-day independence, are Walgreens-owned, and that corporate leadership bears responsibility for coordination failures between the pharmacy and provider sides.
